Agreement of Release and Waiver of Liability

This form covers all classes offered by Youga Yoga including , workshops, retreats, trainings and classes hosted by Youga Yoga. Please read the following and sign the following sheet to agree to the terms and conditions.  

I hereby agree to the following terms and conditions:

That I am participating in Yoga Classes offered by Youga Yoga Donation-Based Studio during which I receive information and instruction about healthy and safe practice. I recognize that these classes may require physical exertion, which may be strenuous and could result in physical injury, and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in classes. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in these classes.  

I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the classes offered with Youga Yoga. I agree that it is my responsibility to inform my instructor/teacher of any physical limitations, physical discomfort and/or injuries before or during classes, and I take full responsibility for nondisclosure. 

In further consideration of being permitted to participate in classes, I knowingly, voluntarily and expressly waive any claim I may have against Youga Yoga and its hosting partnerships for injury or damages that I may sustain as a result of participating in these classes. 

I understand that these events may be photographed or video recorded.  I consent release of all photos and video recordings that I may be included in for public use by, but not limited to, Youga Yoga and it’s hosting partnerships.

I have read the above release waiver of liability and fully understand its contents. I voluntarily agree to its contents. I voluntarily agree to the terms and conditions stated above. 

COVID-19 Release and Waiver of Liability

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Youga Yoga  has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Youga Yoga can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Youga Yoga staff, and other Youga Yoga clients and their families.

I voluntarily seek services provided by Youga Yoga and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.

I attest that:

* I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.

* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold Youga Yoga harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the school and clinic, or that may otherwise arise in any way in connection with any services received from Youga Yoga. I understand that this release discharges Youga Yoga from any liability or claim that I, my heirs, or any personal representatives may have against the school and clinic with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Youga Yoga. This liability waiver and release extends to the Clinic and School together with all owners, partners, and employees.

By providing us your personal information you are agreeing to the terms and conditions of the Agreement of Release and Waiver of Liability and your submission of this information is equivalent to an electronic signature.
